چکیده

Awake craniotomy represents an essential option for surgical procedures requiring a patient’s participation in defining the extent of resection. This includes excision of epileptogenic foci, performed in patients with medically refractory epilepsy. Approximately 2 million Americans with a diagnosis of epilepsy are treated with Antiepileptic Drugs (AEDs), of these 20 percent continue to have seizures accounting for over 75 percent of epilepsy treatment cost in the United States. Surgical intervention can possibly eliminate epilepsy for patients’ refractory to medical treatment; however, only a small percentage of potential candidates are referred to epilepsysurgery centers. Management of anesthesia in awake craniotomy.

UNLABELLED The awake craniotomy technique was originally introduced for the surgical treatment of epilepsy and has subsequently been used in patients undergoing surgical management of supratentorial tumors, arteriovenous malformation, deep brain stimulation, and mycotic aneurysms near critical brain regions. This surgical approach aims to maximize lesion resection while sparing important areas ...

Initial Experience with Brain Mapping under Awake Craniotomy for Resection of Insular Gliomas of the Dominant Hemisphere

Background & Importance: Insular lobe is located at the depth of sylvian fissure and is hidden by frontal, temporal and parietal lobes in close vicinity of internal capsule and basal ganglia and adjacent to the speech centers in the dominant hemisphere. Thus, radical resection of insular gliomas can be even more difficult. Brain mapping techniques can be used to maximize the extent of...

Sedation for Epilepsy Surgery under Local Anesthesia

Performing craniotomy with the patient awake continues to represent an important option for the surgical management of patients with refractory epilepsy when intraoperative functional neurologic testing or electrocorticography (ECoG) are needed to define the extent of the corticectomy. The long duration of many neurosurgical procedures, the need to perform intraoperative functional cortical map...

Anesthetic care during awake craniotomy in a pediatric patients

Cortical mapping has demonstrated a wide variability in the location of areas controlling speech, memory, motor and sensory function. These regions of the cortex, often referred to as eloquent cortex, can be near areas of epileptic foci or pathologic lesions, making the surgical resection of these lesions difficult. Optimal assessment of eloquent cortex function during a craniotomy often requir...

Anaesthetic management for awake craniotomy in brain glioma resection: initial experience in Military Hospital Mohamed V of Rabat

The awake brain surgery is an innovative approach in the treatment of tumors in the functional areas of the brain. There are various anesthetic techniques for awake craniotomy (AC), including asleep-awake-asleep technique, monitored anesthesia care, and the recent introduced awake-awake-awake method. We describe our first experience with anesthetic management for awake craniotomy, which was a c...
